Community Healthcare Support Worker - CRT
The Community Response Team supports discharges from acute hospitals, providing short-term and interim care. The team also offers re-enablement to suitable patients to enhance independence. Previous caring experience is ESSENTIAL.
You will be based at Babington Hospital in Belper, covering the localities of Amber Valley and Erewash. Travel between visits is required; therefore, a full UK driving license and access to a vehicle are essential.
You need to be flexible and committed to caring for and supporting people at home with care needs. Shifts follow a 7-day rota, including weekends and bank holidays. The team works early (08:00 - 14:00) and late (16:00 - 22:00) shifts with set visits scheduled.

Main duties of the job
* Attending allocated visits in the community to provide care and re-enablement as required.
* Using a Toughbook and Systmone to follow care plans and document visit notes.
* Communicating effectively with patients, family members, and carers.
* Escalating concerns to senior staff within the Community Response Team.
* Participating in team meetings and actively contributing to team development and strong working relationships.

Person Specification
Education, Qualifications and training (Essential)
* BTEC Health and Social Care / NVQ level 3 in care or equivalent
* Experience working in a healthcare setting

* Training/experience in palliative care, rehabilitation, or care of older persons
Experience and knowledge (Essential)
* Working in a health or social care environment
* Understanding of risk management and safeguarding responsibilities
