Developmental Community LD Mental Health Nurse

Are you a dedicated and compassionate Mental Health Nurse with a passion for working with individuals with learning disabilities?

We are pleased to offer a Band 5 to Band 6 development opportunity for a motivated nurse to join our specialist service supporting people with learning disabilities and complex mental health needs.

This development post is designed to support your transition from Band 5 to Band 6 through a structured programme of professional development, clinical supervision, and mentorship. You will work as part of a skilled multidisciplinary team, delivering person-centred, holistic care, and promoting recovery, independence, and improved quality of life for individuals with learning disabilities and co-occurring mental health conditions.

The role provides opportunities to enhance skills in care coordination, positive behavioural support, risk assessment, and collaborative working with families and agencies. You will be supported to build leadership and clinical competencies required for a Band 6 role, with regular progress reviews and tailored learning objectives.

This is an excellent opportunity for a nurse committed to making a difference and developing their career in a supportive, specialist environment. Upon successful completion of the development programme and achievement of required competencies, you will progress to a substantive Band 6 position.


Main duties of the job

The Community Nurse assesses, plans, coordinates, and reviews a caseload of adults with learning disabilities, who may have physical and/or mental health needs. They will liaise with Primary and Secondary health care providers, as well as private and voluntary care agencies.

The nurse provides clinical support while promoting the delivery of high-quality, outcome-focused, person-centred services.

This is a generic community care role supporting across Plymouth city localities.

Responsibilities include reviewing eligibility for Continuing Healthcare funding, making professional decisions regarding CHC funding, ensuring adherence to the national framework, and following local policies and procedures.

All Livewell staff are expected to work across a 7-day service.

Qualifications

* Registered Nurse in Learning Disabilities or Mental Health
* Evidence of continuing professional development


Additional Requirements

* Ability to travel across Plymouth
* Full Driving License and access to a car for work purposes
* Willingness to travel and attend to service users across Plymouth and other areas
* Able to problem-solve and be creative in care planning


Knowledge

* Understanding of health and social care political dimensions
* Knowledge of Personalised Care Framework
* Understanding of the Mental Health Act, Mental Capacity Act, Deprivation of Liberty, and reasonable adjustments
* Familiarity with safeguarding processes for adults and children
* Competency in assessment, care planning, and review
* Knowledge of recent developments in learning disability and mental health care
* Understanding of physical health needs in adults with learning disabilities


Experience

* Working in multidisciplinary teams across health and social care
* Experience with people with learning disabilities or mental health issues
* Experience working in primary/community health settings
* Experience mentoring students
* Working across organizational boundaries
