Adults Senior Practitioner - Preparing for Adulthood (PfA) Team Hackney Council is looking for an experienced Senior Practitioner to join the Preparing for Adulthood (PfA) team on a locum basis, starting immediately. This is a fantastic opportunity to support young people with disabilities and complex needs as they transition into adulthood. Role Overview Manage a complex and varied caseload, including high-risk and statutory casesComplete Care Act assessments, contribute to EHC plans, and lead on personalised support planningProvide mentoring, guidance, and practice leadership for Social Workers, NQSWs, and studentsSupport the management team with supervision, audits, and case allocationWork collaboratively with health, education, housing, and provider partnersProduce high-quality reports for court and multi-agency meetingsEnsure practice is fully compliant with the Care Act 2014, Children Act 1989/2004, MCA, and all statutory requirements Requirements Social Work qualification (Degree/DipSW/CQSW)SWE registrationStrong background in children's disability services or adult learning disability services (PfA)In-depth knowledge of the Care Act, MCA, and children's legislationExcellent risk assessment, analytical, and decision-making skillsExperience as a Practice Educator or mentoring staffAbility to travel for home visits and work in a hybrid model What's on Offer Locum assignment at £36.00 per hourStart ASAPSupportive team environment within a forward-thinking boroughFlexible hybrid working To Apply: Get in touch with Jordan Peat, QSW Talent Partner - Commercial Services Trading Ltd is acting as an Employment Business in relation to this vac
