The post holder will participate in the development of the GM SMI Physical Health Team, supporting patients with Severe Mental Illness (SMI) to access annual primary care physical health screens, reducing barriers and providing outreach work. The role includes physical health monitoring and escalating information to senior practitioners.
The post-holder will support patients on the GP-held SMI register, working across primary and secondary care settings to promote continuity of care and effective communication. They will contribute to service development and education programs for nursing students and other disciplines.
Responsibilities include providing annual physical health checks as part of the SMI physical health function, supporting care that addresses mental, physical, spiritual, psychological, and social needs, and referring patients to community services when necessary.
The role involves supporting the day-to-day functioning of health/treatment clinics, monitoring medication effects and side effects, undertaking venepuncture after relevant training, and supporting safe storage of medical equipment.
The post holder will promote health and wellbeing, support multidisciplinary meetings, and assist patients in engaging with their health needs, including arranging appointments and contacting other services. They will work within Trust policies, safeguarding protocols, mental health legislation, and the Care Programme Approach.
Additional duties include supporting patients' access to healthy activities, recognizing and responding to challenging behavior, participating in team meetings, and developing skills in supervision and supporting junior staff and students.
