Job Description:
1. PURPOSE OF JOB:
Assist with all aspects of delivery of care and support for patients in an outpatient setting including:
2. assisting physicians and extenders (NP, PA's)
3. patient assessments and monitoring
4. patient education
5. triage nursing (telephonic and in person)
6. Coumadin management
7. operation of equipment
8. Chronic care management
9. Risk stratifying of patients to identify high risk/ high cost patients and their medical needs
10. Telephonic outreach to patients that have been discharged from the hospital, nursing home, emergency room and/or urgent care
11. Participation in value based office meetings including but not limited to Tandigm
12. Other duties as requested
13. ESSENTIAL FUNCTIONS:
14. Manages the day to day support of patient care with patient assessments, plans, education, implementation, and documentation, coordination of care according to department and hospital standards. Utilize hospital and office based electronic medical records system to obtain and document care, including both clinical support for office visit, electronic communication, and telephone communication
15. Communicates and collaborates with patients, physicians, family members and other health care disciplines in order to achieve excellence in patient care. Maintains competency in communicating electronically. Including but not limited to the management of lab results, telephone triage (screen for urgency and level of care required). Receive calls from Visiting Nurses and other allied health professionals prioritize and communicate with care team as needed.
16. Promotes patient safety through adherence to protocols, policies, procedures, evidence-based guidelines and OSHA Goals.
17. Actively participates in and supports departmental and organizational quality initiatives and goals. Understands and follows office policies and procedures.
18. Maintains clinical competency according to unit-specific standards and licensure guidelines.

QUALIFICATIONS:
19. Education:
20. Graduate of an accredited School of Nursing. LPN required ,RN preferred; Primary Care Medical office experience preferred.
21. Current licensure in Commonwealth of Pennsylvania, Department of State Bureau of Occupational and Professional Affairs and successful completion of appropriate continuing education specific to job requirements
B. Other Skills:
22. Certified in for Healthcare Provider
Excellent interpersonal and team building skills
Knowledge of computer functionality
eClinical Works experience preferred
Knowledge of state law requirements and certifications and CME requirements
Skill in patient triage in person and on the phone
Ability to calculate and administer drug dosages and injections and measure results; trained in IV and phlebotomy and IV.
Skill in initiating appropriate crisis interventions and emergency response.
Knowledge of nursing processes; health care system, structure and functions.
Knowledge of chart/medical record documentation requirements and federal/state laws related to release of health care information
