We are seeking a highly skilled and motivated Registered General Nurse (RGN) to join our team as a Complex Case Manager. This is an exciting opportunity for a dynamic individual with proven experience in complex case management within the community and a strong track record in business development.
As a Complex Case Manager, you will play a pivotal role in managing complex patient cases, ensuring the delivery of safe, effective, and personalised care. You will also take a proactive approach in developing new opportunities, building relationships, and supporting the growth of our services.
Key Responsibilities:
Lead and coordinate the management of complex patient cases in the community, ensuring high-quality, patient-centred care.
Undertake comprehensive assessments and create tailored care plans to meet individual needs.
Work collaboratively with patients, families, carers, clinicians, and multidisciplinary teams.
Ensure adherence to clinical governance, safeguarding, and regulatory frameworks.
Identify and develop new business opportunities, fostering strong partnerships with referrers, commissioners, and stakeholders.
Provide expert clinical input to support service growth and innovation.
Monitor and review outcomes to ensure continuous improvement in care delivery.Requirements:
RGN qualification with current NMC registration.
Demonstrable experience in complex case management within the community setting.
Strong business development experience, with the ability to build and maintain professional networks.
Excellent communication, negotiation, and interpersonal skills.
Ability to work independently and as part of a team, with strong organisational and problem-solving abilities.
A flexible and proactive approach.
Full UK driving licence and access to a car.
Willingness to travel to our Manchester branch once per month.What We Offer:
Salary up to £58,000 per annum.
