Job Description:
An exciting opportunity has arisen for an experienced HCSW to join our Intermediate Care Team as part of the Integrated Community & Support Services (ICASS) in the St Andrews Locality.
The Intermediate Care Service provide rehabilitation to the adult population within the locality. This will be delivered to service users as in-patients, out-patients or within their own home. The primary function of this role will be to work with the Occupational Therapy Service within Intermediate Care to deliver therapeutic treatment programmes to service users in the ward setting or their own homes, preventing hospital admission and supporting early discharge from hospital. Occupational Therapists will provide specialist assessments to this patient group and allocate work to you.
You will have an SVQ Level III in Health and Social Care or equivalent. Experience working as a Physiotherapy and/or Occupational Therapy Support Worker would be desirable. It would be desirable to have experience working with older people as they are the main group of service users referred.
Working as part of a supportive multidisciplinary team you will be expected to carry out ongoing treatment programmes for a mixed caseload of mainly older people. You will have good organisational skills, excellent communication skills and be able to work independently as well as within the team.
There may be occasions where you will be required to work in one of the other community hospital sites. The ability to travel throughout the area is essential as this involves community working.
Public Holiday working is in operation.
