Health Check Practitioner

Reed Wellbeing is a public service provider delivering lifestyle and wellbeing programmes that help people improve their health. Pay Range £26,300 to £29,500. This role is a fixed‑term contract until December2026.
The Health Check Practitioner is responsible for delivering NHS Health Check services to eligible residents across Norfolk in community settings such as workplaces, libraries, and public venues. You will conduct and assess lifestyle questionnaires, perform physical measurements, take finger‑prick blood tests and blood pressure measurements, and motivate individuals to make positive lifestyle changes that support long‑term health and wellbeing.
Key Responsibilities
* Set up health check workstations at various venues, ensuring participant privacy and safety.
* Conduct NHS Health Check questionnaires and point‑of‑care testing in line with Reed Wellbeing policies and procedures.
* Capture results accurately and confidentially on Reed Wellbeing systems.
* Advise participants of their test results and risk of cardiovascular disease, and signpost them to specialist support if required.
* Promote the service across community venues and distribute marketing literature to increase awareness.
* Requires access to own vehicle to travel across Norfolk.
Qualifications & Experience
* Ability to establish effective rapport with programme participants and support behavioural change.
* Understanding or interest in health‑related behaviour change, physical activity, healthy eating.
* Coaching skills to identify solutions and develop knowledge and skills to support behavioural change.
* Demonstrable time‑management and personal organisation skills.
* Willingness to work evenings, weekends and travel across the designated region.
Desirable Criteria
* Experience in health check delivery.
* Experience using coaching skills to improve health and physical fitness.
* Fluency in community languages: Cantonese, Mandarin, Hindi, Urdu, Punjabi, Somali, Vietnamese, Dari, Yoruba, Turkish, French, Spanish, Albanian, Arabic, Bengali, Farsi, Gujarati, Polish, Serbian and Greek.
