Diabetes Nurse Practitioner

This is a senior Nursing/Midwifery role within the organisation and there is an expectation that the post holder will fully participate in the leadership and delivery of high quality patient care within the Care Group.
The post holder will be a practitioner within his/her specialty field of Diabetes and Endocrine conditions. The prime function of the role is to provide clinical expertise in supporting the delivery of high standards of nursing care for patients within his/her care.
Work collaboratively with the multi-professional team to develop and maintain a quality service for patients within the specialty. Provide expert clinical nursing, education and advice in all aspects of the specialty. Facilitate an improvement in the quality of health and outcome of care for this client group. Act as a source of expertise and knowledge for patients, carers and health professionals as well as other agencies involved in this specialty. Work with multi-disciplinary teams both in hospital and the community to provide optimal care and improve quality of life.
The post-holder is responsible for supporting the continuous improvement of quality of services and safeguarding high standards of care by sustaining an environment in which excellence in clinical care can flourish.
Main duties of the job
The post holder within the Diabetes and Endocrine conditions speciality are:
* To provide clinical expertise in nursing care for diabetes patients within their care.
* Provide specialist advice and support to patients, health care professionals and others.
* Be responsible for own caseload and make autonomous decisions.
* Empower patients to make informed decisions.
* Maintain accurate, up-to-date clinical records and other relevant documentation using manual/IT systems.
* Contribute to the development of clinical guidelines and protocols.
* Undertake surveys, audit and research and development, demonstrating clinical excellence with measurable standards and outcomes.
* Work in accordance with the NMC code of professional conduct.
* Maintain a portfolio of relevant up-to-date evidence of clinical practice within his/her specialist field.
* Use clinical supervision and peer group support to underpin personal professional development.
* Take responsibility for driving personal annual appraisal process and annual job planning process.
* Provide expert clinical nursing, education and advice in all aspects of the speciality.
* Facilitate an improvement in the quality of health and outcome of care for this client group.
* Act as a source of expertise and knowledge for patients, carers and health professionals.
* Work with MDT both in hospital and the community.
* Support ongoing improvement of the quality of services and safeguarding high standards of care.

Person Specification
Knowledge & Experience
* Relevant post registration experience at Band 5 or above with more than 12 months experience
* Highly specialist knowledge underpinned by theory and experience relevant to post registration experience at Band 5 or above with experience of management in diabetes
* Experience of multi professional working
* Experience of teaching
* Clinical expertise in the specific role of Diabetes and Endocrine conditions
* Experience in running and managing of nurse led clinics independently
* Experience of leading an audit project cycle
Qualifications & Training
* First level Registered Nurse – NMC current registration
* A qualification in Diabetes or willingness to develop one
* Up to date knowledge of professional issues
* IT literate
* Non‑medical prescribing Teaching certificate
* Evidence of mandatory training updates

* Excellent communication skills including written and presentation
* Ability to deal with highly distressing or emotional circumstances
* Compassionate, demonstrating patient‑centred approach to care
