The post-holder will provide support, based upon assessed needs for patients who are both CPA and Non CPA as per Pennine Care NHS Foundation Trust policy.
You will ensure the highest standard of nursing care provided in accordance with the policies and procedures of Pennine Care NHS Trust.
You will work across hospital and community settings, promoting continuity of care and effective communication. You will participate in the on-going development of the service and in the education programmes of nursing students and other disciplines.
Main duties of the job
An exciting opportunity has arisen for a community support worker role embedded within a busy Community Mental Health Team in Bury. The successful candidate will posses the skills and compassion required to focus on delivering psychosocial interventions with patient with complex mental health and social needs, to support them in their recovery journey. Amongst other duties, the candidate will:
Demonstrate the ability to make a judgement about a patient’s mental health or wellbeing needs and act appropriately to manage needs and risks.
Contribute to the provision of a safe clinical environment and promote the dignity of patients.
Support patients whose health and wellbeing is at risk, acting upon and reporting concerns appropriately.
Support patients to engage in meeting educational and vocational needs. This will include arranging initial appointments and attending with the service user if required.
Assist patients to develop skills for everyday living i.e. budgeting money, shopping, cooking, employment, establishing or improving social networks, accessing local services, establishing housing etc.
Support the planning, delivering and evaluation of care programmes with a relevant professional.
Develop and facilitate group work alongside OT, Mental Health Wellbeing Practitioner, nursing and social work staff.

Detailed job description and main responsibilities
Clinical
The ability to make a judgement about a patient’s mental health or wellbeing needs
and act appropriately to manage needs / risks. (For example, supporting self-care
needs by helping a patient to organise their prescribed medication into a dosette
box, or reporting a concern / risk to a qualified member of staff).
To contribute to the provision of a safe clinical environment and promote the dignity
of patients.
To prioritise workload based upon the patient needs / service demands.
Assess the health and wellbeing of people within the CMHT as directed by the
relevant professional.
To provide health promotion for service user’s in the borough CMHT’s,
understanding the relationship between mental health and physical health. To be
able to enhance a patient’s overall wellbeing and promote parity of esteem.
To support multi-disciplinary meetings and contribute to information sharing and
clinical decisions regarding service users in the care of the CMHT’s.
To support patients whose health and wellbeing is at risk, acting upon and reporting concerns appropriately

Education / Qualification
Essential criteria
* NVQ Level 3 or equivalent
Desirable criteria
* BTEC Higher National Certificate
Experience
Essential criteria
* Experience with people with mental illness, and working with patients with severe mental illness.
* Experience of active participation in personal and professional self development
Desirable criteria
* Experience of Lone Working.
* Experience of providing constructive feedback to peers.
Knowledge
