About the Role: We are currently seeking compassionate and dedicated Support Workers with experience in mental health support to join a person-centred service focused on empowering individuals to live fulfilling and independent lives within the community. This role involves working closely with adults who may face a range of mental health challenges, offering emotional and practical support to help them develop life skills, maintain well-being, and achieve their personal goals. Key Responsibilities: Provide day-to-day support to individuals with mental health needs in accordance with their personalised care plans. Assist with daily living activities such as meal preparation, medication prompts, budgeting, and personal care when required. Support individuals to access community services, social activities, education, and employment opportunities. Build positive and trusting relationships with service users, encouraging independence and confidence. Monitor and report changes in mental health, behaviour, or support needs, ensuring appropriate action is taken. Maintain accurate and timely records of support provided, incidents, and progress notes. Work collaboratively with families, health professionals, and other stakeholders to provide consistent, high-quality care. Promote safeguarding and uphold the rights and dignity of all individuals in your care. Participate in regular team meetings, training sessions, and continuous professional development.
