Paediatric Diabetes Specialist Nurse Full or Part‑time opportunity. We are looking for a passionate and experienced Band 6 Paediatric Diabetes Specialist Nurse to join our dedicated Children's Diabetes Service.
This is a rewarding opportunity to make a genuine long‑term difference in the lives of children and young people living with diabetes, as well as their families and carers.
Working as an integral member of our multidisciplinary team you will provide expert clinical care, education and emotional support across inpatient, outpatient and community settings.
Responsibilities
* Provide clinical guidance and expert knowledge in assessing, implementing and evaluating the needs of paediatric diabetes patients.
* Deliver specialist clinical skills, teaching, education, support and leadership to enable families, carers and professionals in all settings.
* Deliver education and support for children and young people, their families and carers at diagnosis within the acute setting and in the wider community, including education settings.
* Work as an effective team member within the multi‑disciplinary paediatric diabetes team, implementing and attaining criteria to meet Best Practice Tariff (BPT) and the National Service Framework (NSF) for Diabetes, and gathering data for the National Paediatric Diabetes Audit (NPDA).
* Provide cover for the Lead Paediatric Diabetes Specialist Nurse in their absence.
* Collaborate with professionals in primary, secondary, tertiary care and outside agencies (e.g. education, social services, voluntary sector organisations) to optimise diabetes management.
* Provide expert diabetes knowledge and education for staff and students working within children’s settings at GWH.
* Manage own caseload, work autonomously and support the team to provide child/young‑person‑centred care.
* Provide efficient and effective programmes of diabetes education and nursing care to children, young people, their families and carers in the community setting.
* Promote holistic, family‑centred care, providing age‑appropriate information and educational material in suitable format to enable self‑management.
* Work with the diabetes team to provide clinical advice and support to children, young people, their families/carers, liaising with other disciplines and external agencies.
* Facilitate, deliver and evaluate the structured education programme to ensure ongoing teaching takes place for all children and young people and their families/carers.
