Hull Family Practice is seeking an enthusiastic and proactive pharmacist to join our existing team of 2 Pharmacists and 2 Pharmacy Technicians.
This is an exciting opportunity to join a well-established and high-performing practice to improve the service we offer to patients whilst supporting the GPs and having the opportunity to develop.
Main duties of the job Acting within their professional boundaries, you will work collaboratively as part of a multi-disciplinary team to clinically assess and treat patients using your expert knowledge of medicines, addressing public health and social care needs.
You will resolve day-to-day medicine issues, consult with and treat patients directly, and provide support for managing long-term conditions, medication advice, and improved access to health checks.
About us We are a large, friendly PCN serving 38,000 patients in East Hull. Our team includes GP Partners, Advanced Nurse Practitioners, Clinical Pharmacists, Pharmacy Technicians, Practice Nurses, Healthcare Assistants, and administrative staff. We are based in modern, purpose-built health centres sharing facilities with other NHS services, committed to providing excellent patient care and supporting training initiatives.
Job responsibilities Job Summary
Reconcile medicines following hospital and clinic discharges, working with patients and community pharmacists to ensure proper medication management.
Respond to requests for acute medication and manage medicine queries from patients and healthcare professionals.
Improve prescribing quality and safety through audits and adherence to local formularies.
Engage with NHS England and the ICB on healthcare priorities and initiatives.
Implement drug withdrawals and alerts to enhance medicines safety.
Provide expertise in clinical medicines review, addressing public health and social needs.
Work with primary care professionals and patients to implement NICE and other evidence-based guidelines.
Participate in multimorbidity and multidisciplinary reviews.
Provide medicines information and training to practice staff.
Review pathology results for patients on known medicines.
Serve as a medicines information resource for the practice team and patients.
Qualifications & Training
Master's Degree in Pharmacy or equivalent (minimum 4 years) and registration with the GPhC.
At least 2 years postgraduate experience in pharmacy.
Possibly working towards a postgraduate pharmacy qualification.

Plan, manage, and review pharmaceutical care programs.

Use analytical skills to evaluate routine situations and options.
Prioritize problem-solving and refer to GPs when necessary.
Follow legal, ethical, and organizational policies and codes of conduct.
Involve patients in medication decisions and adherence support.
Collaborative Working Relationships
Recognize roles within the organization and communicate effectively with stakeholders.
Work effectively as part of a team and recognize personal limitations.
Liaise with ICB colleagues and other practices to ensure consistent patient care.
Leadership
Understand pharmacy governance and contribute to workplace vision.
Improve quality within service limitations and develop clear improvement plans.
Motivate oneself to achieve goals.
Management
Understand implications of national priorities and resource utilization.
Conform to standards and resolve risk management issues.
Education, Training, and Development
Be a role model and understand mentorship processes.
Conduct teaching and assessment according to learning plans.
Engage in continuous professional development and participate in formal education programs.
Person Specification Qualifications Master's Degree in Pharmacy or equivalent and GPhC registration.
Minimum 2 years postgraduate experience.
Experience Experience in General Practice; postgraduate qualifications or prescribing qualification are advantageous.
