StElizabeth Hospice is an independent charity and hospice, which improves lifefor people in the East Suffolk, Great Yarmouth and Waveney areas living with aprogressive or life-limiting illness.Established in 1989, the hospice has built a reputation for deliveringhigh-level care to patients and the community it serves. Thehospices work is centred on the individual's needs and they provide specialistsupport wherever it is needed; whether that is in the home, another setting inthe community, in hospital or in the hospice itself. Support is provided topatients and their families via the Inpatient Unit, the Community Care Unitincluding outpatients clinics, home visits from the community nursing, medicalor therapy team, the OneCall 24/7 telephone helpline and bereavement support.
Main duties of the job

Our Care Coordinators work alongsideour clinical teams to coordinate patient services in our palliative carecoordination hub. The team covers allaspects of work in the hub including being the first point of contact forcallers to our 24/7 advice line OneCall.Our team cover shifts on a four week fixed rota, 7 days a week includingBank Holidays, with shifts covering from 8am to 8pm. Fulltraining will be provided.
Salary: £24,729.00 to £26,380.00plus enhancements for unsocial hours
