Lead Practice Nurse – Respiratory & Chronic Disease Management
An established and forward-thinking primary care organisation is seeking an experienced and motivated Lead Practice Nurse to join their clinical leadership team. This role offers the chance to lead on the delivery and development of high-quality care for patients with COPD and Asthma, while also playing a key role in wider chronic disease management and QOF performance.
The Opportunity
As Lead Nurse, you will take ownership of the respiratory patient cohort, providing expert clinical care, education, and support to patients and their families. You will work collaboratively within a multidisciplinary team, contributing to service development, clinical leadership, and improved patient outcomes.
This position is ideal for a nurse currently working in primary care chronic disease management who is ready to step into a more strategic and leadership-focused role.
Key Responsibilities
* Deliver specialist nursing care for patients with COPD and Asthma, including assessment, diagnosis, treatment, and ongoing management
* Lead and run dedicated respiratory clinics, utilising evidence-based practice and current national guidelines
* Interpret clinical data, including laboratory results and patient self-monitoring, to inform treatment plans
* Prescribe and manage medications (where qualified), ensuring safe and effective care
* Work autonomously as an advanced practitioner while contributing to team-based care delivery
* Support and lead on QOF performance, working closely with the wider multidisciplinary team
* Provide education and self-management support to patients in collaboration with wellbeing services
* Build strong relationships with patients, carers, and external healthcare professionals across community and secondary care
About You
* Registered General Nurse (NMC registered)
* Proven experience within a primary care setting
* Strong clinical background in COPD, Asthma, and chronic disease management
* Knowledge of NICE guidelines and the Quality and Outcomes Framework (QOF)
* Confident working independently with excellent clinical decision-making skills
* Passionate about patient education, service development, and delivering high-quality care
Desirable:
* Independent Nurse Prescriber qualification
* Qualification or strong experience in diabetes management
