Mental Health Link Practitioners Ayrshire Medical Group are looking for a MHW to join our growing multidisciplinary practice. Can be based in Stevenston Health Centre/West Kilbride Medical Practice
You will have a registered professional qualification with the NMC, SSSC or HPC, and have experience in mental health assessment and care planning with an interest in working with patients with mental health problems in the community and General Practice setting.
Advanced communication skills, well developed risk assessment skills and excellent organisational skills are essential together with evidence of continued professional development.
The candidate may be responsible for a caseload of service users working in partnership with the General Practice team(including our current mental health worker) and health and social care partnership services, service users and voluntary groups to support and underpin a seamless journey through the health and social care system to maintain consistently high standards of service delivery and care in a climate of change and redesign.
