Health Family Practitioner – Lancashire Young People & Healthy Families Service
We are looking for a dedicated and proactive Health Family Practitioner to join our Lancashire Young People & Healthy Families Service. In this role, you will work under the direction of our Health Visitors, School Nurses and Staff Nurses, supporting preschool and primary‑aged children and their families. Your knowledge of child development and commitment to improving health outcomes will help deliver key elements of the Healthy Child Programme and targeted interventions based on need.
This is a part‑time opportunity (30 hours per week).
What You’ll Do
* Deliver delegated mandated health reviews, including Pre‑1‑year and 2‑2½‑year developmental checks, identifying additional needs and referring into specialist services when required.
* Work confidently in community settings, using your own initiative to support families.
