Job Summary
This is an opportunity to join North Cumbria Primary Care (NCPC) as a Practice Nurse based at Warwick Square Group Practice; 20 hours per week. Ideally the candidate will have experience working within General Practice/Primary Care, but not essential. Previous knowledge and experience with the management of a long term condition is advantageous.

Main Duties
Provide and maintain a high standard of nursing care for patients as well as providing nursing assistance to the GPs and other members of the Healthcare team.
* Manage patients with chronic disease including diabetes, vascular and respiratory disease.
* Administer childhood and adult vaccinations.
* Run well‑person clinics and health promotion programmes, providing assessment and lifestyle advice on diet, smoking, alcohol intake and exercise.
* Perform venepuncture and wound management following current evidence‑based guidelines.
* Assist patients to identify their health needs, incorporating use of care plans.
* Perform holistic assessment, cervical cytology smear tests, and contribute to Quality and Outcomes Framework.
* Maintain accurate records and assist with administrative systems in the Practice.
* Attend and participate in any Practice meetings when required.
* On occasion undertake home visits.
Qualifications – Essential
* Registered General Nurse (RGN) or Registered Nurse (RN).
* Minimum 2 years post‑registration experience.
* Knowledge of needs of patients with long‑term conditions and chronic disease management.
* Aware of accountability of own role and other roles in a nurse‑led service.
* Knowledge of health promotion strategies and clinical governance issues in primary care.
* Knowledge of patient group directives and associated policies.
* Evidence of appropriate continuing professional development.
* Proven ability to evaluate the safety and effectiveness of own practice.
Qualifications – Desirable
* Ability to identify determinants of health in the local area.
* Chronic disease diplomas in Asthma, COPD, Hypertension, Diabetes or vascular management.
* Knowledge of public health issues in the local area and awareness of local national health policy.
* Awareness of issues in the wider health economy.
* Knowledge or experience in audit participation and CQC compliance.
