Band 3 Operational Discharge and Flow Support HCSW – Winchester & Basingstoke Sites
Band 3 Operational Discharge and Flow Support HCSW roles are available on the Winchester and Basingstoke sites. The role involves working collaboratively with the Site and Duty team, In‑patient and clinical areas, and Integrated Discharge Teams to support earlier discharges from wards and streamline patient movement from Emergency Departments and assessment areas. The successful candidate will work dynamically with multiple key stakeholders to provide targeted support to alleviate blockages and barriers to patient flow.
Responsibilities
* Discharge coordination with the Integrated Discharge Team, Patient Flow Facilitators, ward teams and the Operations Centre.
* Maintain accurate data on electronic whiteboards (eWhiteboards) and use Power BI dashboards to gather discharge intelligence.
* Proactive planning for same‑day and next‑day discharges, ensuring discharge lounges are operational from 07:30 hrs to facilitate early patient transfers and bed availability.
* Prepare and verify discharge summaries, TTOs and related documentation the day prior to planned discharge.
* Confirm appropriate patient transport is pre‑arranged, encouraging patients to arrange their own transport when clinically appropriate.
* Support patients with breakfast and confirm collection details with relatives/carers.
* Monitor progress of all discharge actions and promptly escalae barriers to the Band 7 lead, Band 5 or Duty Manager.
* Ensure safe and timely transfer of patients from ED/admission areas to inpatient wards and discharge lounges, prioritising patient flow.
* Redeploy rapidly to areas of greatest operational pressure and adapt to evolving site requirements with a proactive "what’s next?" approach.
* Maintain regular communication with the Duty Manager and Site Coordinator to align with operational priorities, and develop strong relationships with key stakeholders across clinical and non‑clinical teams.
* Engage patients identified for discharge lounge transfer, providing clear communication about the process and timelines, and coordinate packing, collection and transfer of patients, property and medications.

Potential candidates must be able to work across all functions of the role and work intuitively under the guidance of the Band 7 and Site and Duty team. Key capabilities include flexibility, responsiveness, and the ability to move quickly and efficiently between tasks while supporting patient engagement and experience.
