Job Summary
We are looking to recruit a motivated, committed and enthusiastic Clinical Pharmacist with previous experience of working in General Practice to join our friendly practice with a patient population of around 4600.
Clinical staff include two GP's, Practice Nurse, Advanced Nurse Practitioner and healthcare Assistant.
Main duties of the job
* Work with the lead GP and Medicines Management team on quality improvement projects.
* Patient facing element (face to face or telephone) – repeat prescription management and structured medication reviews.
* Manage and review patients with poly‑pharmacy.
* Provide specialist pharmaceutical support within clinical competencies, collaborating with the wider healthcare team to improve care and support safe and effective prescribing.
* Involved in quality and improvement and audit, managing aspects of the Quality and Outcomes Framework.
* Proactively support quality and improvement activity.
* Manage aspects of QM and ongoing practice.
* Ensure compliance with Care Quality Commission standards for safe and effective care.
About Us
The Practice is located in Central Rochdale with a tram stop in front of the building and the Rochdale train station nearby. We are housed on the 2nd floor of a Modern Lift Building and are part of the Canalside Network.
Job Description
* Clinical medication reviews for patients with single or multiple long‑term conditions where medicines optimisation is required.
* Reconcile medicines following discharge from hospitals or intermediate care, working with patients and community pharmacists to ensure post‑discharge medication provision and reduce readmission risk.
* Manage repeat prescription requests, resolve queries within scope and ensure appropriate monitoring tests are in place.
* Implement changes to medicines in line with MHRA alerts, product withdrawal or shortage and other local/national guidance.
* Maintain complete records of all patient contacts using suitable clinical templates and coding.
* Proactively undertake audit and quality improvement, implementing recommendations where appropriate.
* Participate in review and setting of policies relating to medicines management and prescribing, ensuring practice prescribing complies with local guidelines and formulary.
* Identify areas of clinical risk and recommend new working practices that optimise prescribing quality and resource utilisation.
* Maintain own clinical and professional competence and manage own continuous professional development.
* Ensure full compliance with Care Quality Commission standards for safe and effective care.
Person Specification
Qualifications (Essential)
* Mandatory registration with the General Pharmaceutical Council.
* Completion of an undergraduate degree in pharmacy and registration with GPhC.
* Clinical Diploma.
* Previous working experience in general practice.
* Demonstrated understanding of and adherence to relevant standards of practice.
* Follows professional and organisational policies/procedures relating to performance management.
Desirable
* May hold or be working towards an independent qualification.
