We are seeking an experienced and motivated Lead PracticeNurse to join our forward thinking and patient focused primary care team. AsLead Practice Nurse, you will play a key leadership role in the delivery anddevelopment of high-quality nursing services within the practice.

You will beresponsible for overseeing the nursing team, developing staff potential,and motivating your team through mentoring, appraisals and learning events. Youwill contribute to quality improvement initiatives and ensure clinicalexcellence.

This is a pivotal role that combines hands-on clinical practice,leadership and service co-ordination. While previous leadership experience is desirable,we also welcome applications from experienced practice nurses who are ready totake the next step in their career. For the right candidate this can be adevelopmental role with support to transition into leadership.


Main duties of the job

Provide clinical leadership & direction to the nursing team, ensuring highstandards of patient care are consistently delivered. Supervise, mentor andsupport the practice nursing team, identifying training and support needs tosupport ongoing professional development.

Coordinate and lead the delivery of a wide range of nursing services, includingchronic disease management (e.g. diabetes, asthma, COPD), immunisations, woundcare, cervical screening, and health promotion.

Ensure compliance with current clinical guidelines, infection control policies,and national/local protocols. Contribute to audits & risk assessments

Deliver holistic, evidence-based care tailored to individual patient needs,ensuring safeguarding, dignity, and respect at all times.

Work collaboratively with GPs, other clinical staff, and external health andsocial care partners to ensure integrated, high-quality care.

Maintain accurate, comprehensive clinical records using appropriate systems (SystmOne),ensuring data protection and confidentiality.

Support the development and implementation of practice policies, protocols, andprocedures in line with best practice and regulatory requirements.

Take a lead role in managing and reviewing patients with long-term conditions,using data and recall systems to support proactive care.

Job responsibilities

Support the development of others to maximise staffpotential in the team.

Identify the potential for service developments, risk anddeficits and inform practice manager making recommendations based on specialistknowledge

Undertake one-to-ones, appraisals, regular Practice Nurseand HCA meetings, clinical education, and other practice-wide learning eventsand meetings.

To provide routine nursing care to patients as required in accordancewith clinical based evidence, NICE and the NSF

As the Clinical lead, you will have overall line managementresponsibility for the Practice Nursing Team

Play a key role in our clinical governance and overallpractice management team.

Act as a clinical leader in the delivery of practicenursing services to patients, ensuring that the needs of the patient are thepriority.

Attend regular practice meetings.

Lead the nursing team in the planning and implementation oflocal guidelines, protocols, and standards, as well as local projects orinitiatives.

Ensure health and safety and infection control are highpriorities, supported by other members of the team.

Work with our Practice Management team to ensure we monitorpractice performance of QOF and LCS regularly to achieve high standards of patientcare.

Take responsibility for own development, learning, andperformance, including organising clinical supervision where appropriate andacting as a positive role model.

Work to support staff wellbeing and development.

Quality improvement.

Work with colleagues and management team to ensure that wefocus our clinical time to maximum benefit of patients and that our systems andprocesses support staff to focus on what matters most.

To administer medication for patients in accordance withPGDs and own scope of practice

To provide guidance to patients in the use of prescribedand over-the-counter medications regarding side effects and dosages

To oversee the provision of wound care services to patients

To co-ordinate the provision of travel medicine services

To request pathology services as necessary

To process pathology results as required

To maintain accurate clinical records in conjunction withexisting legislation

To ensure read codes are used effectively by all members ofthe nursing team.

To chaperone patients where necessary

To prioritise health issues and intervene appropriately.

To support the team in dealing with clinical emergencies

To implement vaccination programmes for adults and children

To liaise with external services/agencies to ensure thepatient is supported appropriately (vulnerable patients etc.)

To delegate clinical responsibilities appropriately(ensuring safe practice and the task is within the scope of practice of theindividual)

To support the clinical team with all safeguarding mattersin accordance with local and national policies

To understand practice and local policies for substanceabuse and addictive behaviour, referring patients appropriately

To deliver opportunistic health promotion whereappropriate.

To be responsible for ensuring maximum optimisation of useof appointments across the nursing team.

Develop and review audit protocols and process for the practice,effectively utilising the audit cycle and to oversee those within the team.


Person Specification


Qualifications

* NMC registration
* Minimum of Degree level education or equivalent
* A minimum of 3-5 years Post Qualification Experience
* Experience of working in a primary care environment
* Experience of working as a practice nurse
* Experience of working autonomously
* Experience of infection prevention and control measures
* Wound care/removal of sutures and staples
* ECGs
* New patient medicals
* Chaperone procedure
* Immunisations - routine, childhood and travel
* Ability to record accurate clinical notes
* Ability to work within own scope of practice and understand when to refer to GPs
* Understanding of infection prevention and control measures
* Knowledge of health promotion strategies
* Chronic Disease Diplomas
* Non-medical prescribing - this could be supported if required
* Understand the importance of evidence-based practice
* Broad knowledge of clinical governance
* Venepuncture
* Requesting pathology tests and processing the results, advising patients accordingly
