An exciting opportunity has arisen for a motivated and enthusiastic individual to join our team as a Social Work Assistant Practitioner (SWAP), working within the Caithness Integrated Health and Social Care Team.
Based in Wick, the successful applicant will work across Caithness, providing care and support to individuals in their own homes and other community settings. The role involves supporting adults with complex needs, including those affected by:
* Learning disabilities
* Substance misuse
* Mental health issues
* Physical illness or disability
* Acquired brain injury
* Dementia and/or old age
Key responsibilities include:
* Completing needs-led assessments and developing person-centred care plans
* Securing funding for care packages
* Working collaboratively with colleagues across health and social care
Essential requirements:
* A full driving licence
* A degree-level qualification in a relevant discipline, or equivalent experience and qualifications appropriate to the role
