Job Overview
To work as an autonomous clinical practitioner to provide expert clinical care for frail, elderly & housebound patients with acute, intensive & complex medical needs. To diagnose, review and treat patients in their own home.
Key Responsibilities
* Provide expert clinical care for frail patients with multiple long‑term conditions and at risk of deteriorating health, preventing avoidable hospital admission or unnecessary length of stay.
* Support and work with carers and wider family members.
* Develop relationships with staff within the Practice, ICT, Rapid Response and Social Care.
* Undertake comprehensive assessment of the physical and psycho‑social care needs of frail patients with complex chronic disease.
* Establish individual functional capabilities regarding frailty and any long‑term condition clinical management.
* Create person‑led evidence‑based holistic health and social care plans in partnership with medical and other health and social care colleagues and with patients and their relatives.
* Use high‑level communication and interpersonal skills for cognitive assessment and mental health status; refer individuals to mental health and/or other services where appropriate.
* Teach and educate patients and carers about early warning signs to facilitate rapid management of complications or crises.
* Enable patients to be as independent as possible through self‑management strategies, supported by the voluntary sector.
* Provide end‑of‑life care and support, including prescribing medication for terminal patient care and liaising with specialist teams.
* Maintain accurate, contemporaneous and complete record keeping of patient consultations.
* Support lifelong learning and shared learning within the practice.
* Identify vulnerable adults and children, keep up to date with statutory child protection training, procedures and protocol.
* Work effectively as part of the practice nursing team, emergency team and overall practice team following values, policies and protocols.
* Manage and assess risk professionally and raise concerns to the nurse manager/general manager when necessary.
* Follow infection control guidelines, equality and diversity policies and ensure absence of discrimination.
Person Specification
Qualified registered prescribing nurse, ideally with matron experience and experience in primary care and working with the elderly.
About Us
You would be part of a friendly & professional team of 5 matrons. The Southgates and The Woottons Surgeries are based in King's Lynn, with a long and proud history of excellent patient care. You will be required to work across both surgeries and may be required to work extended hours.
Contract Details
This role is a permanent contract for 18 hours per week (2 days: Monday & Tuesday). Work may be required across both Southgates and The Woottons Surgeries in King's Lynn.
