We are a friendly, hardworking, and dedicated multidisciplinary group of professionals in the Community Adult Physical Health Directorate who are looking for a Consultant‑grade medical practitioner in any of the following specialisms Acute Medicine, Emergency Medicine, or Geriatrics, or a general medical practitioner (GP) to work to further develop our community model to provide excellent care for our patients as an alternative to unplanned hospital attendance or admission. If you possess expert senior decision‑making skills, enjoy direct patient care, have outstanding communication skills and can lead, work and teach an enthusiastic team, this is the job for you.
The main responsibility of this post is to lead and develop the virtual ward (VW) and Urgent Community Response Service (UCR) and may be required to provide medical support across the range of community services and there will be opportunities to develop your special interest. This job description is for a full‑time 10 PA postholder. Should any applicant wish to undertake a part‑time position, the clinical duties and job plan will be adjusted accordingly. There is no on‑call responsibility attached to this post.
Responsibilities
* Consultant/GP leadership for the Rapid Response team
* Participation in quality assurance and audit projects
* Clinical review of all UCR and VW patients though direct assessment or via team board rounds and multidisciplinary meetings
* Clinical supervision of the ACPs in their teams
* 1.5 PA of supporting professional activities for audit work, service development, structured teaching and CPD
* No on‑call commitments
* The descriptions of the post cannot be exhaustive, and the post‑holder may be required to provide Consultant/GP support across the wider community service provision following discussions with the Clinical Director or Service Director, normally these will be broadly in line with the listed key responsibilities.
