About the role - Carry out or support Care Act assessments, including self-assessments, to identify proportionate, cost-effective options. - Design and implement personalised, outcome-focused support plans that promote independence, choice, and reduce long-term care costs. - Complete risk assessments and Mental Capacity Act assessments, ensuring statutory compliance and safeguarding principles. - Manage a complex caseload in line with legislation, Council policy, and best practice. - Work collaboratively with NHS partners, specialist teams, and third-sector organisations to deliver holistic support. - Engage service users, carers, and advocates throughout assessment, planning, and review processes. - Maintain knowledge of local provision and make timely referrals to specialist services. - Support and promote personal budgets, direct payments, and self-funding options, guiding clients through processes. - Liaise with procurement teams to arrange services, equipment, and adaptations. - Act as a trusted assessor where trained, reducing unnecessary handovers. - Chair and participate in multi-agency meetings and reviews. - Ensure timely reviews of care plans and adapt support to changing needs. - Maintain accurate, up-to-date case records and prepare reports as required. - Promote and safeguard the welfare of adults, adhering to Safeguarding policies and acting as Best Interest Assessor where trained. - Lead in specialist areas (e.g., HIV, asylum, sensory impairment) as appropriate. - Ensure compliance with statutory frameworks, professional standards, and Council procedures. - Contribute to service improvement through evaluation, team meetings, and training. - Work flexibly to meet changing needs and deliver value-for-money solutions within budgetary constraints. About you You must be able to: - Deliver high-quality social work in line with legislation and policy. - Must be registered with Social Work England. - Work within a multidisciplinary team to support vulnerable adults. - Focus on outcomes set out in the Care Act 2014. - Use a person-centred, enabling approach in all aspects of your work. - Carry out structured assessments to design and commission tailored support packages. - Manage complex cases requiring specialist input, including long-term care planning and advanced interventions.
