At The Dovecot and Densham Surgeries, our mission is to deliver the highest standard of compassionate, patient-centred care by working together as a unified team.

We are two small, friendly, independent practices, based in the same building, on the same floor; we are highly supportive and understand the importance of a good work/life balance with an open-door ethos. Whilst we are hardworking, we have informal catch-up coffee meetings, and peer support.

Our whole team across both practices comprises of:

Four GP Partners, one salaried GP, five GP Registrars, five Nursing Team members.

We have sixteen Support Staff, along with a Practice & Deputy Manager.

We have the support of a Practice Pharmacist, via the PCN, who works on site.

We provide front‑line NHS services in Stockton and surrounding areas, to approximately 8,000 patients. We aim to make a lasting positive difference to the quality of care people receive and to support people to be healthier, happier and in control at every stage of their journey.


Main duties of the job

We are seeking a highly motivated and compassionate GP to join our friendly, patient-centred team.

The role is to undertake duties governed by the GMS contract and Objectives of Direct and Local Enhanced Services including:

* Face to face and telephone consultations.
* Patient Home visits and Nursing Home visits.
* Checking and signing repeat prescriptions.
* Administration – dealing with queries, patient test results, referral letters NHS/private, task, emails and other paperwork and correspondence in a timely manner.
* Cover all the clinical management of the patients in surgery as appropriate including initiating investigations, reviewing results, making referrals to secondary care or to other providers as appropriate.
* Develop care and treatment plans in consultation with patients and in line with current Practice disease management protocols as well wider guidance such as that from NICE.

Primary key responsibilities

* The delivery of highly effective medical care to the entitled population.
* The provision of services commensurate with the primary care contract.
* To clinically examine and assess patient needs from a physiological and psychological perspective and plan clinical care accordingly, referring patients to other services/agencies in a timely manner, utilising Practice and any other local guidelines.
* Generic prescribing adhering to local and national guidance.
* Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
* Effective management of long‑term conditions.
* To support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self‑care.
* Processing of administration in a timely manner, including referrals, repeat prescription requests and other associated administrative tasks.
* Review medications following the appropriate policies, NICE/local clinical guidelines and local care pathways.
* Maintain accurate clinical records in conjunction with good practice, policy and guidance.
* Work collaboratively, accepting an equal share of the practice workload.
* Adhere to best practice recommended through clinical guidelines and the audit process.
* Support the delivery of anticipatory care plans.
* Contribute to the successful implementation of continuous improvement and quality initiatives within the practice.
* Accept delegated responsibility for a specific area (or areas) or the QOF, as required.
* Attend and contribute effectively to practice meetings as required.
* Contribute effectively to the development and maintenance of the practice including clinical governance and training.
* Be aware of your duties and responsibilities regarding current legislation and adhere to practice policies and procedures on Safeguarding Adults and Safeguarding Children.
* Ensure compliance with the appraisal process.
* Prepare and complete the revalidation process to remain fit to practice.
* Commit to self‑learning and instil an ethos of continuing professional development across the practice team.
* Review and always adhere to practice protocols and policies.
* Encourage collaborative working, liaising with all staff regularly, always promoting a culture of continuous improvement.
* To be an integral part of the general practice team as well as supporting the wider network and multi‑disciplinary team.
* There may be, on occasion, a requirement to carry out other tasks, this will be dependent upon factors such as workload and staffing levels.


Wider responsibilities

* Participate in the spectrum of clinical governance processes.
* Participate in formal training events promoting best practice in area of expertise. Provide leadership and education for members of the multi‑disciplinary team, providing guidance and support when necessary.
* Participate in local initiatives to enhance service delivery and patient care.


Person Specification Clinical Knowledge & Skills Essential

* Outstanding level of clinical knowledge and skills commensurate with that of an experienced GP.
* Understanding of the importance of evidence‑based practice.
* Good MS Office and clinical system IT knowledge.
* Understanding of the spectrum of clinical governance processes.
* Excellent communication skills (written and oral) including recording of accurate clinical notes.
* Ability to work within own scope of practice and understand when to refer to GP colleagues.
* Experience of triage and the ability to effectively assess and diagnose.
* Ability to listen whilst being honest and caring and be both sensitive and empathetic in distressing situations.
* Understanding of the local pathology tests processes and the ability to processing the results with advising patients accordingly.
* Ability to assess and manage acute deteriorations /exacerbations of long‑term conditions.
* Understanding of safeguarding adults and children.
* Understanding of the importance of evidence‑based practice.


Desirable

* Knowledge of health promotion strategies.
* Experience of medicines management.
* Knowledge of public health issues in the local area.
* Understanding and knowledge of QOF and enhanced service.

Qualifications Essential

* Qualified GP
* MRCGP
* Vocational Training Certificate or equivalent JCPTGP
* Certificate of Completion (CCT)


Eligibility Essential

* Full GMC registration.
* Performers List registration noting that confirmation on this list is adequate for DBS confirmation.
* England conforms to the National Performers List and is able to perform any such service as detailed within Regulation 24 of the National Health Service (Performers Lists) (England) Regulations 2013.
* Scotland Post CCT Performers List
* Wales All Wales Medical Performers List and All Wales Locum Register
* Northern Ireland Primary Medical Performers List (PMPL)
* Appropriate defence indemnity.
* Evidence of current validation.
* Evidence of last appraisal.
* Eligibility to practice in the UK independently.


Experience Essential

* Ability to both work autonomously and as part of a multi‑disciplinary team.
* Experience of working in a primary care environment.
* Experience of providing home visits.
* Minimum of two years as a salaried GP.
* General understanding of the primary care contract.
* Proven ability to evaluate the safety and effectiveness of own clinical practice.


Desirable

* An appreciation of the NHS landscape and relationship between individual organisations, PCNs and the commissioners.


Other requirements and wider responsibilities Essential

* Disclosure Barring Service (DBS) check.
* Occupational health clearance.
* Flexibility to work outside of core office hours.
* Evidence of continuing professional development (CPD) commensurate with the role.
* Access to own transport and ability to travel across the locality on a regular basis.
