We are seeking dedicated Support Workers to join our West Lothian Outreach service. This is a person-centred position, supporting adults with learning disabilities to live independently, achieve meaningful outcomes, and participate in their local community.
Responsibilities
* Providing care and support, including personal care, medication administration, shopping, housework and cooking.
* Assisting individuals to maintain and develop independent living skills.
* Offering social support to promote community involvement and inclusion.
* Supporting the development and maintenance of positive relationships.
* Liaising with medical professionals when required.
Requirements
* Driver’s license/ability to travel across West Lothian with shifts allocated in Polbeth, Broxburn, Uphall Station, Bathgate and Deans (drivers essential).
* An enhanced Adult PVG is essential for this role.
Shift Patterns
* Day Shifts: 8am-3pm, 9am-3pm, 10am-3pm, 11am-3pm
* Afternoon/Evening Shifts: 3pm-9:30pm
* Waking Nights: 9:30pm-8:00am

Role Details
We provide care and support to adults with learning disabilities, with a strong focus on promoting independence, dignity, and quality of life. Support is tailored to each individual and may include personal care, assistance with medication, housework, and meal preparation, as well as encouraging and developing independent living skills. Social support is offered daily and weekly to ensure that each person remains involved and included within their community. All support is guided by personalised support plans and risk assessments, which are regularly reviewed to ensure safety and consistency. Staff liaise with families, medical professionals, and other agencies when necessary to meet each person’s needs. The role also involves devising, following, and implementing risk assessments and support plans to provide accurate, reliable, and person-centred care always.
We work with individuals who may present behaviours that challenge; staff are trained accordingly to ensure a consistent approach. Flexible on shifts, predominantly based from our Polbeth Hub, with 1:1 community support as needed. You must be able to work across West Lothian.
