Overview
We are looking for a pro-active, dedicated and flexible Primary Care Practice Nurse or Registered Nurse to join our professional clinical team. This role will be primarily based at the Lincoln Clinical Diagnostic Centre (CDC) where you will support the nursing and administrative teams in the delivery of clinical and administrative services. There may be opportunities to work across the HLMG GP Practice Sites depending on the candidate.
The role will be delivered and assisted within the Heart of Lincoln Medical Group Primary care multi-disciplinary team, in delivering care within their scope of practice to the patient population where necessary. The role will deliver and assist primarily in Clinical Diagnostic/ Diagnostic Pathways/ Physiological Measurement tests – namely phlebotomy, blood pressure monitoring and/or 24hr BP, ECG taking, ambulatory cardiac monitoring and when trained spirometry/FENO investigations. The role will also act as a liaison for the Head of Patient Care and Clinical Services for all elements related to IPC Clinical Governance and Audit for the CDC.
The candidate will be employed by Heart of Lincoln Medical Group (HLMG) who are responsible for the delivery of physiological diagnostic tests within the CDC on behalf of Lincoln Healthcare Partnership Primary Care Network (PCN). CDCs provide a broad range of elective diagnostic services away from main acute hospitals, offering easier access to tests and greater convenience to patients. There are CDCs operational in Grantham and Skegness. Planned interview dates: 26th September 2025 and 29th September 2025.

Job description
Primary responsibilities
The following are core responsibilities of the role. There may be occasions to carry out other tasks depending on workload and staffing levels:
* a. Undertake Physiological Measurements for the CDC and undertake clinical tests needed for any Clinical Pathways within the CDC
* b. Support the practice nurse with health promotion programmes
* c. Carry out baseline observations such as pulse oximetry, blood pressure, temperature and pulse rate, recording findings accurately
* d. Facilitate routine and 24-hour BP monitoring, advising patients accordingly
* e. Undertake wound care, dressings and other clinical tasks as required
* f. Support the practice nurse with the management of chronic disease clinics
* g. Carry out BMI checks as directed
* h. Act as a chaperone as required
* i. When trained, undertake venepuncture
* j. When trained, administer flu vaccinations
* k. When trained, carry out ear irrigation under the supervision of the practice nurse
* l. Carry out ECGs as requested
* m. Ensure specimens are recorded and ready for onward transportation
* n. Provide support during minor operations as required
* o. Ensure all clinical rooms are adequately stocked and prepared for each session
* p. Ensure fridges are cleaned routinely in accordance with extant guidance
* q. Ensure clinical waste is removed from clinical areas and sharps bins replaced in accordance with the organisation’s IPC policy
* r. Deliver opportunistic health promotion where appropriate
