The East Riding OT Service is seeking a proactive, person‑centred Occupational Therapist to support adults living across our communities to remain safe, independent and well.
In this role, you’ll carry out functional assessments, provide early intervention, and develop tailored, strengths‑based plans that promote independence and reduce risk.
You’ll contribute to front‑door triage, offer timely advice, and recommend equipment, rehabilitation programmes, and minor or major adaptations that enable people to live confidently at home.
You will also work across a variety of community settings, including prisons, and support individuals to maintain autonomy through informed decision‑making.
You’ll collaborate closely with health, social care, housing and voluntary partners, sharing information effectively and contributing to joint assessments and MDT discussions.
High‑quality documentation, professional reporting, and adherence to HCPC standards, safeguarding and information governance are essential parts of the role.
You’ll also have opportunities to contribute to service development, engage in supervision and CPD, and support assistants and students within the team. Please note: this summary is not exhaustive.
To be successful, you’ll need to be an HCPC‑registered Occupational Therapist with experience of functional assessments, moving and handling, and equipment or adaptation provision. Strong communication, clinical reasoning and organisational skills are essential, along with a full driving licence and access to a vehicle. Experience in community settings, reablement, adult social care, specialist equipment, DFG processes or Blue Badge assessments would be beneficial, alongside confidence working as part of a multidisciplinary team.
