As a Clinical Pharmacist, you will provide a comprehensive and patient-centred Clinical Pharmacy service to patients at Bupa Cromwell Hospital. Your responsibilities will include participation in the antibiotic stewardship round, MDT input, therapeutic drug monitoring, medicines reconciliation, and dispensary support. You will be responsible for delivering safe and effective pharmaceutical care and promoting best medicines management practices.
What you'll do
1. Provide a comprehensive and patient-centred Clinical Pharmacy service, including participation in antibiotic stewardship, therapeutic drug monitoring, medicines reconciliation, and dispensary support.
2. Facilitate and promote rational and optimal medicines usage.
3. Provide relevant medicines information to healthcare professionals and patients.
4. Monitor medicines management on wards/departments, including Controlled Drugs, and advise nursing staff on legal control, safe custody, and security of drugs.
5. Validate prescriptions in the dispensary to ensure appropriateness for the patient, following procedures and good practice.
6. Offer medication counselling to patients to ensure safe and effective use of medicines.
7. Participate in ward rounds and clinical hand-overs, promoting the role of the Pharmacist as an integral part of the multidisciplinary clinical team.
Qualifications and Experience
* Registered with the General Pharmaceutical Council.
* Postgraduate diploma in Clinical Pharmacy, with a CPD Portfolio, and working towards RPS faculty accreditation.
* Experience in outpatient pharmacy and managerial experience in a hospital environment.
