Overview
We are seeking a dedicated and compassionate Advanced Nurse Practitioner to join our healthcare team. The ideal candidate will possess a strong commitment to patient care and demonstrate expertise in clinical practices. As a Nurse Practitioner, you will play a critical role in assessing, diagnosing, and managing patient health issues, ensuring high-quality care and support for our patients, demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for unscheduled patients presenting to general practice.
The post holder will work with the wider practice team to delivery high quality care, with continuous evaluation and quality improvement by developing innovative concepts, models and practices to meet the changing needs of our practice.
You will work with the management team to develop services and implement tests of change aimed at realising the practice development plan and the RCGP 2022 vision for General Practice.
The post-holder is a key member of the Annat multi disciplinary team, through which Advanced Nurse Practitioners will be required to deliver care that was previously provided only by GPs.
Responsibilities
* Conduct thorough patient assessments, including taking medical histories and performing physical examinations.
* undertaking telephone consultation and advice calls as well as participate in the daily house visits to patients.
* Develop and implement patient care plans based on clinical findings and patient needs.
* Administer medications as prescribed and monitor patients for adverse reactions or side effects.
* Collaborate with clinicians and other healthcare professionals to provide comprehensive care.
* Educate patients and their families about health conditions, treatment options, and preventive measures.
* Maintain accurate and up-to-date patient records in compliance with healthcare regulations.
* Stay current with medical advancements and best practices in nursing care.
Skills
* Strong knowledge of anatomy and physiology to effectively assess patient conditions.
* Proficient in medication administration, including understanding pharmacology principles including v300.
* Excellent communication skills to interact with patients, families, and healthcare teams effectively.
* Ability to work independently as well as part of a multidisciplinary team.
* Strong organisational skills to manage multiple tasks efficiently while maintaining attention to detail.
