Are you a talented and patient-focused clinician looking to make a difference? Our client is seeking a Specialist Physiotherapist to join their passionate team, based in Southampton. This community-based role offers the opportunity to deliver personalised care in patients' homes, facilitating early supported transfers from hospital or preventing unnecessary admissions.
Responsibilities
* Conduct comprehensive holistic assessments of patients.
* Formulate individualised physiotherapy treatment plans.
* Deliver physiotherapy sessions in home or community settings to maximise patient independence.
* Educate patients and carers on the correct use of mobility and functional equipment.
* Travel regionally to visit patients at home, requiring a full UK driving licence.

* Annual salary up to £39,000.

Qualifications
* Previous community experience or a track record of working autonomously.
* Experience in an acute care setting.
* HCPC registration.
* Strong IT skills.
* Willingness to cover three different shift patterns, 5 days out of 7.
* A full UK driving licence, comfortable with driving in and around Southampton.
