We’re looking for dynamic, forward‑thinking Band 5 Community Staff Nurses to join our Community Nursing Team within the Integrated Nursing and Conditions Services at CHCP CIC.
As part of our integrated community nursing team, you’ll deliver high‑quality, holistic care to people in their own homes, helping them stay well, safe, and supported in their community.

* Passionate about providing excellent community‑based nursing care

* Community nursing experience is helpful, but not essential—we’ll support your development
* Able to travel across the locality with access to suitable transport
