Job summary
As a Funded Nursing Care Assessor you will play a key role assessingand reviewing people's eligibility for NHS Funded nursing care in line with The National Framework for NHS ContinuingHealthcare and Funded Nursing Care (FNC) and where appropriate, referring onwardsfor a Continuing Healthcare assessment.
Main duties of the job
A Funded Nursing Care assessor willplay a key role in supporting the Personal Health Commissioning (PHC) service anddelivering an effective and competent level of service and consistentlydelivering a client-focused service.
You will work within an establishedsupportive team, completing assessments face to face or virtually.
You will be assignedapproximately four nursing homes and will have the opportunity to liaise and workclosely with nursing home staff to ensure initial assessments and reviews areundertaken in a timely fashion and ensuring a good standard of care is beingdelivered. There will be liaison with patients, families, providers andpotentially other professionals within the health and social care sectors.

The post holder will play a key role in supporting the Personal HealthCommissioning (PHC) service across the region. Working within the remit of TheNational Framework for NHS Continuing Healthcare and Funded Nursing Care (FNC),the post holder will deliver an effective and competent level of service andconsistently delivering a client-focused service.
The post holder will assist andsupport in the facilitation of patient reviews in line with standard operatingpolicy and procedures and ensuring The National Framework for NHS ContinuingHealthcare and Funded Nursing Care (FNC) is implemented and complied with,according to the principles and processes.
The post holder should hold a full UK driving licence andhave access to a vehicle to be able to fulfil the requirements of this post.

Qualifications
Essential
17. Educated to degree level in relevant subject or equivalent level of experience of working at a similar level in specialist area, ideally Continuing Healthcare
18. Nursing qualification or Allied Health Professional Occupational Health, Physiotherapy etc. or registered Social Worker
19. Knowledge of NHS Continuing Health Care and associated procedures and practices, underpinned by theoretical knowledge of the National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care and Childrens and Young Peoples Continuing Care
20. Evidence of Continued Professional Development

21. Extensive knowledge of specialist areas acquired through experience and training plus further specialist knowledge of the Personal Health Commissioning process
22. Knowledge, understanding and ability to implement the Personal Health Budget process
