We are looking to recruit an experienced Team Manager for the Hospital Integrated Discharge Team (HIDT), based at the Queen Elizabeth Hospital SE18 4QH.
As Team Manager, you will lead and manage a staff team to ensure we are meeting statutory and locally agreed obligations and to ensure compliance with all relevant legislation. As Team Manager you will also be working to improve the efficiency and effectiveness of the Team. This will involve contributing to business plans, policy and practice review and development, while ensuring the departmental vision is embedded in practice.
The team support the discharge of Greenwich residents from acute hospital settings and work collaboratively with other professionals to resolve complex situations. We work closely with NHS partners, other local authority colleagues and internal directorates to ensure safe and timely discharges, which can sometimes be challenging due to competing priorities and demands.
We are seeking a candidate with proven experience of managing a team and ensuring strength based practice is followed at all times. You will be ensuring the team is undertaking relevant and timely assessments of need and mitigating risk including Safeguarding cases.
Applicants must have a social work qualification and be registered with Social Work England.
You should have excellent interpersonal skills and be able to complete written work to a very high standard whilst working in a fast-paced environment.
The successful candidate will need substantial knowledge of adult social care legislation, principles and best practice, and be cognisant of the issues facing older people and people with disabilities.
