New Road SurgeryBromsgrove is looking for an experienced and enthusiastic professional to joinour multidisciplinary team as an Advanced Care Practitioner (ACP).
We would welcome applicants from a nursing or other allied health professional background.
The role is advertised at 0.5 whole time equivalent. However, we are looking to employ someone for between 2 - 3 days a week, there is some flexibility to this which can be discussed at interview.
Main duties of the job
The successful candidate will undertake both emergency minor illness, and routine clinics within the surgery as part of our advanced practitioner team.
We would welcome applicants who have experience in either home visiting, or a clinical specialty for example women's health.

Job responsibilities
Main Responsibilities:
Clinical Practice:
To deliver a highstandard of patient care within the practice, using advanced autonomousclinical skills and a broad, in-depth theoretical knowledge base.
Working as a memberof the routine and duty clinical team consisting of GP, ANP/ACP and Paramedic, managing aclinical caseload, dealing with patients presenting needs within a primarycare setting.
Work as anautonomous practitioner, providing clinical care including telephone and faceto face, triage, diagnosis, and treatment including home visits, as appropriate.
Utilise advancedskills and expert knowledge to assess,diagnose, plan, implement & evaluate treatment/interventions and care forpatients presenting with an undifferentiated diagnosis.
Clinically examineand assess patient needs from a physiological and psychological perspective,and plan care accordingly.
Assess, diagnose,plan, implement & evaluate interventions/treatments for patients withlong-term conditions or complex needs.
Diagnose and manageboth acute and chronic conditions, integrating both drug and non-drug-basedtreatment methods into a management plan and within scope of practice.
Initiate andinterpret tests and investigations as appropriate.
Prescribe andreview medication for therapeutic effectiveness, appropriate to patient needsand in accordance with evidence-based practice and national and practiceprotocols, and within scope of practice.
Work with patientsin order to support compliance with and adherence to prescribed treatments.
Provide informationand advice on prescribed and over the counter medication on medicationregimens, side effects and interactions.
Support patients toadopt health promotion strategies that promote healthy lifestyles and applyprinciples of self-care.
Participate invaccination and immunisation programmes for both adults and children whererequired.
Prioritise healthproblems and intervene appropriately to assist the patient in complex, urgentor emergency situations, including initiation of effective emergency care.
Record accuratedata in the patient records to an agreed standard.
Collaborate withother members of the Primary Health Care Team including doctors, nurses andother AHPs, accepting referrals and referring to them for specialist care.
Direct referral toand liaison with consultants and other professional agencies.
Ensure a highstandard of clinical practice is maintained by ensuring care is based oncurrent best practice, embarking upon further training if required andcontinuing to update clinical knowledge.
Quality:
Recognise and workwithin own competence and professional code of conduct as regulated by the NMC / HCPC.
Produce accurate,contemporaneous, and complete records of patient consultation, consistent withlegislation, policy, and best practice.
Be aware of andfollow data protection regulations (GDPR) and confidentiality requirements.
Use technology andappropriate software as an aid to management in planning, implementation andmonitoring of care, presenting, and communicating information.
Review and processdata using accurate Read codes in order to ensure easy and accurate informationretrieval for monitoring and audit processes.
