Hyndburn and Rossendale Community Mental Health Team are seeking to appoint a Band 6 Community Mental Health Practitioner to a fixed term post. An opportunity has arisen for a Nurse, Social Worker or Occupational Therapist to join the Hyndburn and Rossendale Community Mental Health Team. You must be a motivated individual, committed to promoting recovery and well-being for our service users as part of our multidisciplinary team. Excellent communication skills are essential. Previous experience of Community Mental Health work is desirable.
This post provides the opportunity for a forward thinking and experienced individual to develop and implement safe, sound and supportive services that embrace the provision of holistic care. With a strong emphasis on multidisciplinary working and the principles laid out in the Recovery Model, you will need to be an innovative thinker and be able to form effective relationships. You must be a registered mental health practitioner, with excellent leadership and clinical skills. You must have some post qualification experience in clinical work with service users experiencing a range of mental health problems. You need to be able to work as part of a multidisciplinary team and have experience of planning, implementing and reviewing care needs. You will also need to have a working knowledge of the Care Programme Approach (CPA), Dialog+, risk assessment and management, and the ability to explore issues relating to the impact of health and social needs on mental health and well-being.
