Responsibilities
* Assist in the provision of a high standard of planned individualized, holistic rehabilitation programmes for patients in the community, under the care of the Urgent Care Response and Intermediate Care Team, including recuperative and palliative care.
* Support the management and discharge of patients as part of the rapid response team, working in the community as required.
* Be prepared to work over seven days per week to meet service needs.
* Support general practice in early identification and anticipatory care planning for all patients identified at risk of admission to hospital in order that inappropriate admissions are avoided.
* Assist in providing nursing/therapy care as delegated by the Lead Therapist/Team Sister within patients' homes, including short‑term personal care such as washing, dressing and meeting all hygiene needs prior to a care package commencing.
* Maintain knowledge, skills and attitudes necessary to deliver a quality service that is responsive to the changing needs of patients.
* Participate in and support qualified team members in health promotion activities.
* Provide clerical and administrative support to the team as delegated by the Lead Therapist/Team Sister, using strong communication and organisational skills to deliver positive patient experiences at every stage of their healthcare journey.
* Safeguarding and protecting the children, young people and vulnerable adults that we work with is of utmost importance. We have policies and procedures in place to promote safeguarding and safe working practices, and everyone who joins the team is subject to a safer recruitment process, including disclosure of criminal records and vetting checks.
Qualifications
* Own car with a valid driver’s licence to travel across services in a timely manner.
* Confidence using IT systems and ability to demonstrate working knowledge of Microsoft Office packages (Outlook, Word, Excel).
* Strong organisational and communication skills with a flexible approach to deliver a high quality service.
