Job purpose: Claimont Home Care Ltd is a CQC-registered, unique service in the private sector delivering specialist mental health nursing at home, especially when inpatient admission is being considered or for inpatients treated in hospital but who wish, and are able, to be discharged home. Claimont is not an agency. We invest in people and build relationships based on mutual support and long-term commitment. Our work is varied and rooted in a patient-centered, holistic approach. We value our nurses' and support workers' skills and experience and support them with any additional training needed.
We are seeking high-quality, dynamic, and experienced Mental Health Support Workers to provide waking night and/or day support as part of our bank staff team.
Key accountabilities
* Support individuals in their care, ensuring adherence to management policies, protocols, and practice guidance, updating as needed based on national and local guidance.
* Maintain written records of all aspects of client treatment and progress, including electronic patient records within 24 hours of an event.
* Assist clients in accessing community life through activities.
* Work with families and carers to help them understand mental health issues and cope effectively.
* Maintain confidentiality in all treatment delivery.
* Communicate effectively with referring agents and team members.
* Maintain high professional psychiatric skills and develop clinical links with colleagues, carers, relatives, and community services.
* Encourage evidence-based practice and contribute to team meetings.
* Report assessment findings appropriately and ensure data confidentiality and security in line with legal requirements.
* Attend mandatory training and engage in personal development.
* Perform additional duties as assigned by the Registered Manager to ensure quality service delivery.
* Participate in performance reviews and develop personal development plans.
* Ensure professional standards are maintained and mandatory training is current.
* Uphold the principles and values outlined in the Claimont Contractors Manual.
* Prioritize patient health and wellbeing in all activities, promoting safety and quality of care.
* Adhere to equality, health and safety policies, and statutory regulations, reporting incidents as necessary.
* Comply with information governance, confidentiality, safeguarding, and recruitment policies.
* Support safeguarding initiatives to protect vulnerable populations.
* Maintain effective communication within the team and with external stakeholders, representing Claimont positively.
