Responsibilities
Are you a confident nurse with community experience who wants to work with patients in an acute setting to maximise their independence? An excellent opportunity has arisen for an enthusiastic and motivated nurse to join Locala’s new Self-Management Service.
The self-management service works towards the NHS’ Long-Term Plan for personalised care. Patients will be reviewed both at home and virtually while working towards becoming experts in their own healthcare interventions.
Your main role will be to support learning and competence for patients, ensuring they feel confident and able to optimise their skills, maximising the benefits of self-management. You will be based primarily at Huddersfield Royal Infirmary.
The successful candidate will:
* Be a nurse with community experience who is passionate about patients’ choices, their abilities to self-manage, and personalised care.
* Build effective working relationships with colleagues in the acute setting, developing processes and pathways that encourage the shift from hospital to home care.
* Be willing to undertake additional clinical skills and health coaching training required for the role.
* Act as a role model, using their skills to support patients in an acute setting, building confidence so they feel able to continue with these skills at home.
* Demonstrate flexibility in approach, allowing for regular evaluation and adaptation of the service during its early stages.
* Utilise support services throughout Kirklees to provide personalised, high-quality care, ensuring better patient outcomes and positively influencing service change.
