Outreach Support Worker Hull - Full Time & Part Time Hours
We are seeking an Outreach Support Worker to join our team in Hull. Our vision is ‘A life without limits for the people we support’ which reflects our culture of a knowledgeable, passionate and dedicated workforce.
Our Outreach Service provides 1:1 support to people experiencing mental health needs, Substance use, Learning disabilities and or Autism who live in the community, as well as people in hospital or care settings who are moving back into the community, and require support to build networks prior to returning.
The successful candidate will have experience in Mental Health, Learning disabilities or Autism, although this is desirable, not essential. A driving licence is also desirable but not essential.
Main duties of the job
* Prepare person-centred support plans with clients which reflect their views and aspirations and respect their beliefs and opinions, reviewing and amending them as necessary and considering the client’s changing needs and wishes.
* Support clients to fulfil their aspirations, supporting them to live as independently as possible, in accordance with their skills and abilities.
* Communicate in an open and transparent way with clients and other team members, acting with integrity and doing what you say that you will do.
* Provide personal care to clients in a way that respects their choices and dignity, if required.
* Keep appropriate, accurate records, including daily notes, with full involvement of the client.

Detailed Job Description And Main Responsibilities
As an outreach Support Worker, you will provide high quality 1:1 care and support to our clients to enable them to live their best life. You will be passionate in making a difference to our client’s lives, helping them to fulfil their aspirations and ambitions and live as independent as possible.
* Ensure that all support and care are provided in a way that protects clients from abuse and exploitation, maintains their dignity, and supports positive risk-taking.
* Support quality inspection processes, cooperating with on-site inspectors in relation to information requests.
* Work always in accordance with Health and Safety requirements, such as Manual Handling, Food Hygiene, Fire Safety, infection control, and Risk Assessment procedures.
* Carry out a range of household duties in accordance with the needs of the client including cooking, cleaning, laundry, and other domestic duties as required.
* Willing to provide an appropriate level of care and support to clients, in accordance with their care plan including personal care.
* Understanding of the importance of maintaining accurate written and numerical records
* Ability to undertake a wide range of personal care and domestic duties (specific to each service)
* Able to carry out the physical requirements of the role including but not limited to climbing stairs/using hoists/pushing wheelchairs/moving people/night work.
* Understand or willing to learn how to use computer packages (such as Word, Outlook and the Internet) and to undertake relevant training and qualification in care if required.
* Able to work cooperatively as a team member.
* Able to liaise and communicate effectively with carers, families, inspectors and other external stakeholders.
