You will join a longstanding team of committed and passionate colleagues working in a person-centred way to meet the needs of the people we support. You will work within Care Dorset’s Community Care Strategy for Residential and Day Care Services to provide a wide range of support and opportunities to maintain and promote the independence and quality of life for service users. Key Responsibilities Support the manager / assistant manager in the delivery of services to a variety of Service User groups in a day centre or community setting. Act as a keyworker and undertake the assessment of service users needs in accordance with policies and procedure with the involvement of other professionals and families as appropriate. Undertake risk assessment and develop care plans and individual service plans to meet individual service user needs and needs identified in the community care plan for approval by senior staff. Implement, monitor and review individual service plans Supervision of individual and group programmes/courses. Provide personal care to Service Users, as required, toileting, washing and the administration of medication within guidelines. ix. Undertake day to day supervision and direction of other staff e.g. care assistants, catering staff and volunteers, as required. To undertake the assessment of service users and the development implementation and review of care plans without close supervision from senior staff exercising initiative within the context of case accountability remaining with the manager. To work without close supervision outside the base establishment with access to senior staff as required. Undertake any other duties of a lesser or comparable nature as required by the Managing Director. To undertake specialised training to facilitate the care and activity programme. Knowledge and application of relevant legislation e.g., Care Standards Act 2000, Community Care Act 1991. Knowledge of the risk assessment process Knowledge of care and support planning processes Demonstrable supervisory/management experience Experience of care planning and review process Experience of undertaking risk assessments, including their application and review Experience of understanding and applying anti-discriminatory practice Ability to relate to a wide range of people and understand their needs The Successful Candidate Must Have Good general standard of education English GCSE at grade C or above, or equivalent Basic knowledge of Health and Safety Relevant legislation – NHS and Community Care Act, Valuing People 2001 Valuing People 2007 Health and Voluntary agencies Advocacy Person Centred Planning Substantial experience of working with people with complex needs and who present challenges in their behaviour Substantial experience in a caring profession Experience of working with people with profound and multiple learning disabilities Good standard of written and recording skills Good To Have Social work qualification NVQ3 Health and Social Care Knowledge of Care Dorset’ s policies, practices and procedures Knowledge of Direct Payments, In Control, Personalised Budgets Facilitating and motivating individuals and groups within Centres and Community Experience of group work Experience of staff supervision Experience of using Person Centred Thinking Tools Some specialist skills i.e. independent living skills Care Dorset is committed to safeguarding and promoting the welfare of vulnerable adults and children and expects colleagues and volunteers to share this commitment.
