About the Role
We're looking for a Care and Support Worker to join our dedicated Reablement Service on a 26.25-hour per week contract. This essential service operates 7 days a week, 52 weeks a year, delivering high-quality care to people in their own homes.
You'll be part of a team that helps individuals regain independence after illness, injury, or hospital discharge, supporting them to live safely and confidently at home.
Working Pattern
1. 4-week rolling rota
2. AM shifts: 7:00am - 3:00pm
3. PM shifts: 2:30pm -10:30pm
4. Includes weekends and bank holidays
5. Driving licence and access to a vehicle required
About the Service
Our Reablement Service offers short-term, person-centred care to people with disabilities or those recovering from illness. We focus on:
6. Promoting independence
7. Relearning daily living skills
8. Providing dignity and respect
9. Working collaboratively with professionals and agencies
We also offer:
10. Bridging care while long-term packages are arranged
11. Resettlement support to help people adjust after time away from home
12. Emergency response care during times of urgent need
Training & Qualifications
Essential:
13. GCSE Grades D$úG in Maths and English
14. Functional Skills Level 1 in Maths and English
Desirable (but not required):
15. Level 2 in Health & Social Care (or equivalent)
16. Don't have the Level 2 qualification? No problem!
We'll provide full training and fund your qualification after successful completion of your probation period.
