We have an exciting opportunity for a Band 5 enthusiastic, motivated, and compassionate nurse to join our established multidisciplinary Care Community Team to support and deliver a range of quality nursing care to our community patients.

This post is 22.5 hours - 12 months fixed term.

Our care community team is based in Tattenhall, covering rural West Cheshire and surrounding geographic area. It’s important that you can drive and have access to your own vehicle for work. But of course, mileage expenses are reimbursed in addition to your hourly rate.
As a Community Staff Nurse the successful candidate will undertake duties between the working hours involving the delivery of high-quality nursing services and the holistic assessments of patients ensuring a person-centred approach.
You will work alongside various professionals to provide quality and seamless care to patients predominantly in their own home. You will be proactive in coordinating their care needs as appropriate, avoiding unnecessary admission to hospital or long-term care settings.
Responsibilities
* Deliver high-quality nursing services and holistic assessments with a person-centred approach.
* Coordinate care needs with other professionals to support patients at home.
* Provide care predominantly in patients’ own homes and work to avoid unnecessary hospital admissions or long-term care placements.
Qualifications and requirements
* Band 5 qualified, enthusiastic, motivated, and compassionate nurse.
* Ability to work within an established multidisciplinary Care Community Team.
* Full driving license and access to own vehicle is preferred due to location in rural West Cheshire.

Additional information
All new starters will commence in post on one fixed start date each month, in line with our Prepare to Care (P2C) induction programme date. P2C offers up to 3 weeks training to support you to meet the induction / initial training requirements of the role.
