An exciting opportunity has arisen to join our dynamic andforward-thinking Pharmacy Team! Based across our network of 6 practices withinthe Primary Care Network (PCN), you'll become an integral part of a supportiveand collaborative team of Pharmacists and Pharmacy Technicians. Main duties of the job

In this role, the post holder will work as an integral part of a multi-disciplinary team, contributing both administratively and in direct patient-facing interactions.

A key responsibility of the post holder will be conducting clinical medication reviews to proactively manage patients with complex polypharmacy, ensuring their treatment regimens are both safe and effective.

The post holder will serve as a primary point of contact for general practice staff, providing expert advice on prescription and medication queries. They will play a crucial role in supporting the repeat prescription system, handling acute prescription requests, conducting medicines reconciliation during transfers of care, and implementing systems for safer prescribing. Additionally, they will offer expert clinical guidance on medicines, addressing the public and social care needs of patients.

Job responsibilities

Keyduties and responsibilities

* Patient facing Long term condition clinics (whereappropriate).
* See (where appropriate) patients with single ormultiple medical problems where medicine optimisation is required (e.g. COPD,asthma, Diabetes).
* Review the on-going need for each medicine, areview of monitoring needs and an opportunity to support patients with theirmedicines taking ensuring they get the best use of their medicines (i.e.medicines optimisation).
* Make appropriate recommendations to SeniorPharmacists or GPs for medicine improvement.

Telephoneor Patient facing Clinical Medication Review

* Undertakeclinical medication reviews with patients and produce recommendations forsenior clinical pharmacist team leader, nurses and/or GP on prescribing andmonitoring.

Telephoneor Patient facing care home medication reviews

* Undertakeclinical medication reviews with patients and produce recommendations for thesenior clinical pharmacist, nurses or GPs on prescribing and monitoring. Workwith care home staff to improve safety of medicines ordering andadministration.
* Domiciliaryclinical medication review
* Undertakeclinical medication reviews with patients and produce recommendations for thesenior clinical pharmacists, nurses and GPs on prescribing and monitoring.Attend and refer patients to multidisciplinary case conferences.

Managementof common/minor/self-limiting ailments

* Managing caseload of patients withcommon/minor/self-limiting ailments while working within a scope of practiceand limits of competence.
* Signposting to community pharmacy and referringto GPs or other healthcare professionals where appropriate
* Provide patient facing clinics for those withquestions, queries and concerns about their medicines in the practice
* Telephone medicines support
* Provide a telephone help line for patients withquestions, queries and concerns about their medicines.

Medicineinformation to practice staff and patients

* Answers relevant medicine-related enquiriesfrom GPs, other practice staff, other healthcare teams (e.g. communitypharmacy) and patients with queries about medicines.
* Suggesting and recommending solutions.
* Providingfollow up for patients to monitor the effect of any changes

Unplannedhospital admissions

* Review the use of medicines most commonlyassociated with unplanned hospital admissions and readmissions through auditand individual patient reviews.
* Put inplace changes to reduce the prescribing of these medicines to highrisk patient groups.
* Managementof medicines at discharge from hospital
* To reconcile medicines following discharge fromhospitals, intermediate care and into care homes, including identifying andrectifying unexplained changes and working with patients and communitypharmacists to ensure patients receive the medicines they need post discharge.
* Set upand manage systems to ensure continuity of medicines supply to highrisk groups of patients (e.g.those with medicine compliance aids or those in care homes).

Signposting

* Ensurethat patients are referred to the appropriate healthcare professional for theappropriate level of care within an appropriate period of time e.g. pathologyresults, common/minor ailments, acute conditions, long term condition reviewsetc.

Repeatprescribing

* Produce and implement a practice repeatprescribing policy.
* Manage the repeat prescribing reauthorisationprocess by reviewing patient requests for repeat prescriptions and reviewingmedicines reaching review dates and flagging up those needing a review.
* Ensurepatients have appropriate monitoring tests in place when required.

Riskstratification

* Identificationof cohorts of patients at high risk of harm from medicines through pre-preparedpractice computer searches. This might include risks that are patient related,medicine related, or both.

Servicedevelopment

* Contributepharmaceutical advice for the development and implementation of new servicesthat have medicinal components (e.g. advice on treatment pathways and patientinformation leaflets).

Informationmanagement

* Analyse,interpret and present medicines data to highlight issues and risks to supportdecision making.

Medicinesquality improvement

* Undertakeclinical audits of prescribing in areas directed by the GPs, feedback theresults and implement changes in conjunction with the practice team.
* Implement changes to medicines that result fromMHRA alerts, product withdrawal and other local and national guidance.
* Implementation of local and national guidelinesand formulary recommendations
* Monitor practice prescribing against the localhealth economys RAG list and make recommendations to GPs for medicines thatshould be prescribed by hospital doctors (red drugs) or subject to shared care(amber drugs).
* Assist practices in seeing and maintaining apractice formulary that is hosted on the practices computer system.
* Auditing practices compliance against NICEtechnology assessment guidance.


Person Specification


Qualifications

* Bachelor's degree in Pharmacy,
* Registered with the General Pharmaceutical Council (GPhC)
* Commitment to undertaking the CPPE pathway training, followed by Independent Prescribing qualification.
* CPPE (or equivalent) trained or already on the pathway


Experience

* Experience in working with computerised health care systems.
* At least 1 year post-qualification experience in General Practice, hospital or community pharmacy.
* Previous experience of working in triage or an unscheduled care service.
