Are you a dynamic, confident and motivated Mental Health Nurse/Occupational Therapist or Social Worker with excellent communication skills with the ability to assess and lead on clinical decision making.

This is an exciting role, working in a fast paced working environment, within a well-established, multi-disciplinary team. The crisis and home treatment teams offer a high quality assessment and home treatment service as an alternative to hospital admission, to individuals experiencing acute mental health crisis.
As a service we are looking for individuals who are genuinely warm and engaging and focussed on client / customer satisfaction.
Main duties of the job
* To provide first line assessments of people experiencing acute mental health difficulties and act as gatekeepers to acute admissions.
* To be able to rapidly assess people with mental health problems whom are vulnerable to hospital admission
* To provide specialist crisis assessments looking at the focus being on "hospital at home".
* To co-ordinate the Crisis and Home Treatment Team on a shift by shift basis, acting as Duty Worker to triage referrals, plan the shifts activities and team accordingly.
* To participate in team MDMs.

* Current registration with the relevant professional body, eg NMC, HCPC, Social Work England
