Job overview
This is an ideal role for a pharmacist wanting to work in Nutrition and Intestinal Failure in a nationally designated specialist centre. This role would enable you to develop your clinical knowledge and skills and to work with a very motivated wider multidisciplinary team for both inpatients and homecare patients.
Main duties of the job
* Advise on medicines management for nutrition support patients.
* Actively participate as a member of daily inpatient PN ward rounds that take place on different hospital sites (primarily at the Churchill and John Radcliffe sites).
* Liaise with appropriate nursing and/or medical staff for vascular access device insertion if needed.
* Act as a non-medical prescriber for PN patients.
* Attend and support nutrition MDT meetings, for both inpatients and outpatients.
* Offer advice on the management of home PN patients to the nutrition team and other hospital staff, as well as to patients directly.
* Liaise and negotiate service provision with external providers for homecare PN.
* Report and investigate adverse incidents related to nutrition support.
* Assist the Lead and Deputy Team Pharmacists in financial and commissioning reporting for the intestinal failure service.
* Participate in SHIFNET and/or BPNG and/or other local/national groups related to PN provision.
* Provide teaching regarding PN to pharmacy, nursing, medical staff, and other allied healthcare professionals (AHPs).
