Job Title: Care CoordinatorLocation: Watton, NorfolkJob Overview:We are seeking a dedicated and organised Care Coordinator to support the delivery of high-quality, person-centred care for residents. In this role, you will work closely with the care team to coordinate and manage residents care, ensuring their individual needs, preferences, and goals are met.Key Responsibilities:
* Coordinate and oversee daily care activities for residents.
* Administer and accurately record medication.
* Maintain and update residents computerised care plans.
* Liaise with healthcare professionals and the wider care team.
* Organise shifts and ensure effective team coordination.
* Support the induction, training, and supervision of care staff.
Skills & Attributes:
* Experience supervising or leading a care team.
* Strong communication and leadership skills.
* Empathy and understanding of residents needs.
* Excellent attention to detail with strong organisational skills.
* Flexible and able to adapt to changing schedules and responsibilities.
Qualifications:
* NVQ Level 3 in Health & Social Care (or currently working towards it).
