Patient Pathway Co-ordinator (Dental Access Portal)

This is a fixed term post/secondment until 31/03/2027. If you are a Hywel Dda University Heath Board employee and are applying for this post as a secondment, agreement for the secondment is needed from your current manager prior to application.

As part of our on-going commitment to the delivery of high quality, reliable services for our patients, an exciting opportunity has arisen in the Primary Care Dental Team within Hywel Dda University Health Board.


Main duties of the job

The post holder will facilitate patient flow through routine and urgent dental care pathways via the Dental Access Portal. The post holder will be supporting patients with queries regarding the Portal and liaising with NHS dental practices and the wider Dental Services Team to ensure that patients are allocated in a timely and efficient manner.

The successful candidate will be expected to demonstrate excellent organisation skills and be flexible/adaptable in their approach to work. They must be able to work on their own initiative, use tact and diplomacy where appropriate and possess excellent communication skills.


About us

Hywel Dda University Health Board plans and provides NHS healthcare services for people living in Carmarthenshire, Ceredigion, Pembrokeshire, and bordering counties.

We have over 13,000 staff and together we provide primary, community, in-hospital, mental health and learning disabilities services.

We work in partnership with the three local authorities, as well as public, private and third sector colleagues, including our valued team of volunteers.

Our services are provided in:

* Four main hospitals: Bronglais Hospital in Aberystwyth; Glangwili Hospital in Carmarthen; Prince Philip Hospital in Llanelli; and Withybush Hospital in Haverfordwest
* Five community hospitals: Amman Valley and Llandovery hospitals in Carmarthenshire; Tregaron Hospital in Ceredigion; and Tenby and South Pembrokeshire hospitals in Pembrokeshire
* Two integrated care centres: Aberaeron and Cardigan in Ceredigion, and several other community settings
* 47 general practices (six of which are health board managed practices); 38 general dental practices and 4 four orthodontic providers; 97 community pharmacies; 43 general ophthalmic practices; and 8 ophthalmic domiciliary providers and
* Numerous mental health and learning disabilities services

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

The ability to communicate in Welsh is desirable for this post. If you do not meet the Welsh Language requirements specified, the Health Board offers a variety of learning options and staff support to help you meet these minimal desirable requirements during the course of your employment with us.

Person Specification


Qualifications and Knowledge

* GCSE Maths and English and Level 3 vocational qualification or equivalent experience
* Knowledge of Medical / Healthcare Terminology
* Knowledge of Patient Pathway
* Knowledge of NHS national policies and strategies
* Knowledge of patient pathways within Dental Services


Experience

* Experience of a full range of duties working in an administrative / office environment
* Experience of working within a multi-disciplinary team
* Experience in use of Microsoft Office (outlook, excel, word, teams)
* Experience in dealing with the public
* Experience of working in NHS
* Experience of working in a pathway administration role


Other

* Welsh Speaker (Level 1)

£24,433 to £26,060 a yearper annum pro rata
