Job Summary

The Integrated Community Rehabilitation Services (ICRS) in South Manchester Local Care Organisation are recruiting Band 3 Rehabilitation Assistants into the Community Rehabilitation Team. The successful candidate should be passionate about providing care to residents in their own homes. Although based in one service, there will be opportunities to work with various services that make up the ICRS, including the Community Therapy Team, Community Falls, Post Covid, and Community Neuro Rehab Team.

ICRS provides therapy for patients with a wide range of conditions who may struggle to access outpatient care. The successful candidates will work with Physiotherapists, Occupational Therapists, and the wider Multi-Disciplinary Team to provide treatment and interventions as prescribed by qualified clinicians.

The successful applicants should have experience working within health or social care teams, excellent communication skills, and a dedication to high-quality patient care.

Applicants MUST have a UK driving license and access to a vehicle for home visits across South Manchester. Evidence of this will be required; without it, you will not progress to interview.


Main duties of the job

* Work with patients with complex medical conditions.
* Assist in delivering and evaluating evidence-based home exercise and therapeutic programs under supervision.
* Collaborate with the team to develop and maintain clinical skills.
* Undertake delegated tasks from qualified healthcare professionals without direct supervision.
* Support patients in achieving goals, promoting independence and healthy lifestyles.

Person Specification

Qualifications

* Essential: GCSE/O’ levels in numeracy and literacy.
* Desirable: NVQ II or above in health-related fields.

Experience

* Essential: Understanding of older people's needs, team collaboration, and experience in health or social care settings.
* Desirable: Experience working with older adults/frailty.

Knowledge and Skills

* Ability to teach practical skills, move and handle patients, communicate effectively, and maintain confidentiality.
* Basic IT skills, ability to work within a team, and awareness of policies and legislation.
* Observation skills for monitoring patient progress.
* Desirable: Experience of lone working and exercise delivery.
